Case Manager - follow-up and outcomes (San Antonio, TX)

We are growing our team at Northwind Lantern Property Management Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 72 people based in San Antonio. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for lease renewals. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with lease renewals. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in San Antonio. • We will tell you what to expect at each step of the hiring process.
