Case Manager - follow-up and outcomes (Salt Lake City, UT)

Silverline Vista Community Partners is a Salt Lake City charity working with residents on basic needs and opportunity. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Salt Lake City. • Applications are reviewed on a rolling basis.
