Case Manager - follow-up and outcomes (Salt Lake City, UT)

Redstone Summit Community Partners is a nonprofit in Salt Lake City supporting families with food, housing and job help, and we need a Case Manager to lead our follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Help with applications and appointments for donor records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Salt Lake City. • We will tell you what to expect at each step of the hiring process.
