Case Manager - follow-up and outcomes (Sacramento, CA)

We are growing our team at Westbrook Yarrow Civic Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 76 people based in Sacramento. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for constituent requests. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with constituent requests. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Sacramento, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
