Summit Yarrow Health Group is looking for a Case Manager. The job centres on follow-up and outcomes for insurance authorisations, and it would suit someone who likes steady, practical work. Continuing education is supported. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Sacramento. • Applications are reviewed on a rolling basis.
