Case Manager - follow-up and outcomes (Sacramento, CA)

Cedar Bridge Care Services Partners supports families and individuals in Sacramento with personal care and coordination. We are hiring a Case Manager to take on follow-up and outcomes. The work is personal and so is the culture. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Sacramento. • We will tell you what to expect at each step of the hiring process.
