Case Manager - follow-up and outcomes (Sacramento, CA)

Bright Pike Learning Group is an education provider in Sacramento serving students and families, and we need a Case Manager to lead our follow-up and outcomes. Families are partners. What you will do • Help with applications and appointments for school calendars. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Sacramento, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
