Case Manager - follow-up and outcomes (Sacramento, CA)

Northwind Willow Property Management looks after rental homes across the Sacramento area, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for vendor invoices. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Sacramento. • Applications are reviewed on a rolling basis.
