Case Manager - follow-up and outcomes (Rochester, NY)

Foxglove Pine Community Partners is a Rochester charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. Flexibility and kindness are expected. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • On site in Rochester, with some flexibility after the first few months. • We reply to every applicant.
