Case Manager - follow-up and outcomes (Rochester, NY)

Redstone Birch Care Services is looking for a Case Manager. The job centres on follow-up and outcomes for visit scheduling, and it would suit someone who likes steady, practical work. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Rochester, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
