Case Manager - follow-up and outcomes (Richmond, VA)

Amberfield Glen Health Partners is a healthcare provider in Richmond with clinics and support teams. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Full-time, Monday to Friday. • On site in Richmond. • We will tell you what to expect at each step of the hiring process.
