Case Manager - follow-up and outcomes (Richmond, VA)

Westbrook Lakeside Care Services Group supports families and individuals in Richmond with personal care and coordination, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Richmond. • We will tell you what to expect at each step of the hiring process.
