Case Manager - follow-up and outcomes (Richmond, VA)

We are growing our team at Stonegate Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 24 people based in Richmond. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client intake. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Richmond, with some flexibility after the first few months. • We reply to every applicant.
