Case Manager - follow-up and outcomes (Reno, NV)

We are growing our team at Clearpath Wilder Property Management and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 185 people based in Reno. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for unit turnovers. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • On site in Reno, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
