Case Manager - follow-up and outcomes (remote)

We are growing our team at Redstone Junction Housing and adding a Case Manager whose main responsibility will be follow-up and outcomes. Collaboration with local agencies is constant. What you will do • Connect clients with partner agencies. • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with quarterly visits in Jacksonville. • Applications are reviewed on a rolling basis.
