Case Manager - follow-up and outcomes (remote)

We are growing our team at Stonegate Summit Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for meal programmes. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with meal programmes. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Fully remote, with an occasional day on site in Virginia Beach. • Applications are reviewed on a rolling basis.
