Case Manager - follow-up and outcomes (remote)

Summit Pike Learning Group runs classrooms and tutoring across the Portland area. We are hiring a Case Manager to take on follow-up and outcomes. We follow the school calendar and we like people who like learning. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for learning materials. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with learning materials. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Portland. • We will tell you what to expect at each step of the hiring process.
