Case Manager - follow-up and outcomes (remote)

Amberfield Birch Care Services Group provides in-home and community support in Columbus. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for four months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Columbus. • We will tell you what to expect at each step of the hiring process.
