Case Manager - follow-up and outcomes (remote)

Silverline Timber Property Management manages apartment communities around Fresno, and we need a Case Manager to lead our follow-up and outcomes. Maintenance and leasing staff share information every day. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for unit turnovers. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Fresno. • We will tell you what to expect at each step of the hiring process.
