Case Manager - follow-up and outcomes (remote)

We are growing our team at Ironwood Pine Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Wichita. • We reply to every applicant.
