Case Manager - follow-up and outcomes (remote)

Summit Thistle Learning Group is an education provider in Rochester serving students and families. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 81 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with an occasional day on site in Rochester. • We reply to every applicant.
