Case Manager - follow-up and outcomes (remote)

Bluepeak Timber Property Management manages apartment communities around Birmingham. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 155 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for resident events. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with resident events. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Birmingham. • Applications are reviewed on a rolling basis.
