Case Manager - follow-up and outcomes (remote)

Fernwood Sable Early Learning is looking for a Case Manager. The job centres on follow-up and outcomes for classroom supplies, and it would suit someone who likes steady, practical work. What you will do • Help with applications and appointments for classroom supplies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with classroom supplies. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Memphis. • We will tell you what to expect at each step of the hiring process.
