Case Manager - follow-up and outcomes (remote)

We are growing our team at Bright Basin Health Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with referrals. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in Little Rock. • We will tell you what to expect at each step of the hiring process.
