Case Manager - follow-up and outcomes (remote)

We are growing our team at Summit Ridge Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 46 people, working mostly from home. What you will do • Help with applications and appointments for licensing records. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with licensing records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Sacramento. • Applications are reviewed on a rolling basis.
