Case Manager - follow-up and outcomes (remote)

We are growing our team at Foxglove Summit Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Indianapolis. • Applications are reviewed on a rolling basis.
