Case Manager - follow-up and outcomes (remote)

Fernwood Meadow Community Partners Group runs community programmes across Anchorage. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 925 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for donor records. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Anchorage. • Applications are reviewed on a rolling basis.
