Case Manager - follow-up and outcomes (remote)

We are growing our team at Rivermark Summit Property Management and adding a Case Manager whose main responsibility will be follow-up and outcomes. Residents are neighbours, not numbers. What you will do • Connect clients with partner agencies. • Help with applications and appointments for vendor invoices. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with a monthly team day in Minneapolis. • Applications are reviewed on a rolling basis.
