Case Manager - follow-up and outcomes (remote)

We are growing our team at Silverline North Property Management and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 34 people, working mostly from home. What you will do • Help with applications and appointments for rent collection. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with rent collection. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Buffalo. • We reply to every applicant.
