Case Manager - follow-up and outcomes (remote)

Summit Stone Care Services supports families and individuals in Madison with personal care and coordination, and we need a Case Manager to lead our follow-up and outcomes. The work is personal and so is the culture. What you will do • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with a monthly team day in Madison. • We will tell you what to expect at each step of the hiring process.
