Case Manager - follow-up and outcomes (remote)

Clearpath Beacon Early Learning operates neighbourhood childcare around Jackson. We are hiring a Case Manager to take on follow-up and outcomes. Low staff turnover is a goal we take seriously. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Jackson. • We reply to every applicant.
