Case Manager - follow-up and outcomes (remote)

We are growing our team at Nova Timber Early Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 46 people, working mostly from home. What you will do • Help with applications and appointments for classroom supplies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with classroom supplies. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Fully remote, with quarterly visits in Portland. • We will tell you what to expect at each step of the hiring process.
