Case Manager - follow-up and outcomes (remote)

We are growing our team at Clearpath Rook Housing Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Housing is our mission and residents are our neighbours. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for repair requests. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Contract for three months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Boston. • We will tell you what to expect at each step of the hiring process.
