Case Manager - follow-up and outcomes (remote)

We are growing our team at Summit Pike Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Fully remote, with an occasional day on site in Portland. • We will tell you what to expect at each step of the hiring process.
