Case Manager - follow-up and outcomes (remote)

Highbridge Meadow Community Partners runs community programmes across Madison, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 119 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for volunteer shifts. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with volunteer shifts. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Madison. • Applications are reviewed on a rolling basis.
