Case Manager - follow-up and outcomes (remote)

Lumen Haven Early Learning is an early-learning provider in Oakland. We are hiring a Case Manager to take on follow-up and outcomes. We celebrate small wins and steady routines. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for licensing records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Oakland. • Applications are reviewed on a rolling basis.
