Case Manager - follow-up and outcomes (remote)

Stonegate Juniper Health Group provides primary and specialty care in Hartford. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for insurance authorisations. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Hartford. • Applications are reviewed on a rolling basis.
