Case Manager - follow-up and outcomes (remote)

We are growing our team at Clearpath Willow Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 25 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Buffalo. • We reply to every applicant.
