Case Manager - follow-up and outcomes (remote)

Clearpath Quill Health Group runs clinics and patient services around Fort Worth. We are hiring a Case Manager to take on follow-up and outcomes. We are busy and kind, and we look after each other. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, afternoons. • Remote-friendly, with a monthly team day in Fort Worth. • We will tell you what to expect at each step of the hiring process.
