Case Manager - follow-up and outcomes (remote)

Greenridge Rowan Health runs clinics and patient services around Minneapolis. We are hiring a Case Manager to take on follow-up and outcomes. Continuing education is supported. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Minneapolis. • We reply to every applicant.
