Case Manager - follow-up and outcomes (remote)

Rivermark Granite Learning Group operates schools and learning programmes around Pittsburgh. We are hiring a Case Manager to take on follow-up and outcomes. We follow the school calendar and we like people who like learning. What you will do • Help with applications and appointments for school calendars. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with school calendars. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Pittsburgh. • We will tell you what to expect at each step of the hiring process.
