Case Manager - follow-up and outcomes (remote)

We are growing our team at Stonegate Drift Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Birmingham. • We will tell you what to expect at each step of the hiring process.
