Case Manager - follow-up and outcomes (remote)

Silverline Stone Care Services is looking for a Case Manager. The job centres on follow-up and outcomes for licensing, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Help with applications and appointments for licensing. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Richmond. • Applications are reviewed on a rolling basis.
