Case Manager - follow-up and outcomes (remote)

We are growing our team at Northwind Sable Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. We celebrate small wins and steady routines. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for meal programmes. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Colorado Springs. • We reply to every applicant.
