Case Manager - follow-up and outcomes (remote)

Lumen Quarry Care Services supports families and individuals in Jackson with personal care and coordination. We are hiring a Case Manager to take on follow-up and outcomes. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with a monthly team day in Jackson. • We reply to every applicant.
