Case Manager - follow-up and outcomes (remote)

Lumen Birch Care Services is a social-care provider serving older adults and people with disabilities around Omaha. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 900 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Omaha. • We will tell you what to expect at each step of the hiring process.
