Case Manager - follow-up and outcomes (remote)

We are growing our team at Foxglove Yarrow Civic Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for permits. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with permits. Working arrangement • Contract for three months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
