Case Manager - follow-up and outcomes (remote)

Bright Willow Early Learning is looking for a Case Manager. The job centres on follow-up and outcomes for meal programmes, and it would suit someone who likes steady, practical work. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for meal programmes. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with meal programmes. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Remote-friendly, with quarterly visits in Houston. • We reply to every applicant.
