Case Manager - follow-up and outcomes (remote)

Highbridge Works is a Toledo property manager with leasing, maintenance and resident services. We are hiring a Case Manager to take on follow-up and outcomes. We are flexible about schedules. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for rent collection. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with rent collection. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with a monthly team day in Toledo. • We reply to every applicant.
