Case Manager - follow-up and outcomes (remote)

Fernwood Loom Property Management is a Phoenix property manager with leasing, maintenance and resident services. We are hiring a Case Manager to take on follow-up and outcomes. We are flexible about schedules. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for rent collection. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with rent collection. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Phoenix. • Applications are reviewed on a rolling basis.
